There's an authenticated scanning system (not optional) that can no longer connect to my CentOS 6 virtual hosts. I can SSH in just fine. All the logs tell me is:
Failed password for user <username> from <IP address>
I need to know a little more than that. I don't know if this is an sshd issue or a PAM issue, but I need more information about WHY this is failing. The account is not locked or expired.

Hi,
The reason behind this is offering multiple keys to the server as the server rejects any key after too many keys are offered. For instance, if you have more than 3 or 4 failed login attempts your IP address might be blocked in server firewall or you can also check the following discussion thread if the issue still persists -
