setuproot: error mounting /proc: No such file or directory
setuproot: error mounting /sys: No such file or directory

CentOS 5 is now End of Life and no more updates to it will be released. You should migrate to a supported release ASAP.

Did you try repairing the RAID from a rescue boot?
The future appears to be RHEL or Debian. I think I'm going Debian.

CentOS 5 and 6 are deadest, do not use them.
