This is a Centos 7 VM running under Hyper-V.
The problem is easily reproducible. I have copied the VM to a different physical server and the problem remains.
The OS was stable for over 1.5yrs. This seems to happen when I trigger any significant activity. Sometimes just during a YUM upgrade.
I've performed the steps for XFS repair (booting with a rescue media,) to no avail.
This is brutal. I guess I will create a new Centos 7 VM and configure the same.
Any thoughts appreciated. Thanks!

You'll probably get better results with the XFS mailing list. See http://xfs.org/index.php/XFS_FAQ#Q:_Wha ... problem.3F
There was a (possibly related) issue of copying an NTFS formatted drive to XFS on CentOS 5 (see https://bugs.centos.org/view.php?id=4425)
