CentOS 7 Reboot fails occasionally with kernel panic

Every once in a while I reboot and it fails with the following issue seen in the image below. Kernel is 3.10.0-862.9.1.el7.x86_64, CentOS Linux release 7.5.1804 (Core).

Nothing in messages except the logs stop until I hard reboot.

The console has the below, note the "systemd-shutdow:1 blocked for more than 120 seconds" and the repeating stack trace having to do with remounting something.

I was on CentOS 7.4 and this issue has persisted after the upgrade. Any ideas or solutions? This has been driving me crazy lately and my Google foo has failed me.

Some additional information:
All logs at /var/log all quit shortly after the reboot is issued, and they contain nothing of interest related to this.
journalctl contains nothing useful as by default it does not save data from prior boots so it was lost at the hard reset. I have since remedied this.
The system is a VM on a 3rd party OpenStack platform, I don't control the drives, they are SSDs, and I have another CentOS VM that I haven't seen exhibit this behavior but was created over a year before this new one and is rarely rebooted so it's possible I just haven't encountered the issue on it.

bump. I really need to know why the kernel crashes at reboot :/
